UST facility and tank forms on file with the EHD are not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD within 30 days of receiving this report. Complete a separate tank form for each tank. 21. Liquid was observed in the under dispenser containment for dispenser 1/2. The designated operator recorded liquid in the UDC for dispenser 1/2 for 2/22/11, 1/25/11,12/16/10,11/26/10, 10/26/10. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the UDC for dispenser 1/2 is maintained free of liquid. 23d. The designated operator employee training for Mandeep Dua, Balveer Benipal and Chandni Dua was not performed for the current year (2011). The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD within 30 days of receiving this report. Notes Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, within 30 days of receiving this report. Provided free UST class information schedule for 2011. The VMI leak detector on the 87 turbine was adjusted to pass the 3 gallon per hour test at 10 psi. The diesel spill bucket had a loose swivel joint - technician tightened and retested to pass the spill bucket. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
