UST facility and tank forms on file with the EHD are not current. Any change of information on these <br /> forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a <br /> copy of the facility and tank forms to the EHD within 30 days of receiving this report. Complete a <br /> separate tank form for each tank. <br /> 21. Liquid was observed in the under dispenser containment for dispenser 1/2. The designated operator <br /> recorded liquid in the UDC for dispenser 1/2 for 2/22/11, 1/25/11,12/16/10,11/26/10, 10/26/10. If water <br /> could enter into the secondary containment by precipitation or infiltration, it must be removed and <br /> disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title <br /> 22 hazardous waste regulations, and manage it accordingly. Ensure that the UDC for dispenser 1/2 is <br /> maintained free of liquid. <br /> 23d. The designated operator employee training for Mandeep Dua, Balveer Benipal and Chandni Dua <br /> was not performed for the current year (2011). The designated operator shall train facility employees for <br /> which he or she is responsible in the proper operation and maintenance of the UST system once every <br /> 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD within 30 days of receiving this report. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, within 30 days of receiving this report. <br /> Provided free UST class information schedule for 2011. <br /> The VMI leak detector on the 87 turbine was adjusted to pass the 3 gallon per hour test at 10 psi. <br /> The diesel spill bucket had a loose swivel joint - technician tightened and retested to pass the spill <br /> bucket. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br /> 1 <br />
