Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />A recent review of the underground storage tank file for the above referenced facility indicated this office has <br />not received this years testing and paperwork requirements. <br />The following information is overdue: <br />CCR 2637(b) Annual monitoring certification <br />CCR 2336(f)(4) Annual line tightness test (If applicable) <br />HSC 25284.2 Annual spill bucket/containment test <br />CCR 2630(d) Annual line leak detector test <br />Most forms may be accessed from our web site address, ivictic.geocities.cont/writiii <br />In addition, this office must be notified of any test date and time within 48 hours of performing the test. <br />Furthermore, results from the testing must be sent to this office within 30 days of the test completion.
