Current Unified Program Consolidated Forms not on current forms. Facility Form A and Tank Form <br />B are on outdated forms. Forms can be located at http://www.sigov.org/ehd under section CUPA. <br />Correct by submitting both Facility Form A and Tank Form B (one per tank) to SJC EHD within 30 days <br />(07-08-10). <br />#5. Monitoring and Response Plans not on current forms. Both plans are on outdated forms. Forms can <br />be located at http://www.segov.org/ehd under the section CUPA. Correct by submitting both Monitoring <br />and Response Plans on current revised forms to SJC EHD within 30 days (07-08-10). <br />#18. Spill container is not tight. The 87 tank spill bucket had failed the half hour Caldwell test. The leak <br />in the spill bucket caused the testing water to leak into the fill sump. Spill container shall collect any <br />hazardous substances spilled during product delivery operation to prevent the hazardous substances <br />from entering the subsurface environment. Correct by having a properly licensed, trained and certified <br />contractor repair/ replace the failed component under permit by SJC EHD. <br />Submit to SJC EHD by 07-08-10: <br />1. one copy of the completed 'Return to Compliance Certification' form <br />2. a statement correcting the above violations <br />3. requested or supporting documents <br />Notes: <br />1. Last years UST Inspection noted the secondary containment testing last conducted 12-11-07 - this is <br />an incorrect date.
