Reporting Format: <br />Annual monitoring system certification report; reporting of audible or visual alarm conditions to the local implementing <br />agency, as well as the responsible party for maintenance, listed under item #6. <br />8. State the preventive maintenance schedule for the monitoring equipment. <br />Annual certification of the electronic monitoring system, as recommended by the manufacturer, and repairs are made <br />when notified by station operator. <br />In the event of a test failure, tank is taken out of service and proper reporting procedures are followed. <br />9. Describe the training needed for the operation of both the tank system and the monitoring equipment. <br />The operator has been trained to check the system daily for proper operation and record the date checked on the <br />"Monitoring System Check Log." All alarms are to be recorded on the "Monitoring System Alarm Log" and reported to <br />the responsible party for Maintenance listed under item #6. <br />We have read, completed and understand the foregoing information regarding implementation of this monitoring program <br />for the UST(s) located at this facility, and we will monitor the UST(s) accordingly. <br />Operator: James J.
