UST Inspection Report 3. UST facility and tank forms on file with the EHD are not current. Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD within 30 days of receiving this report. Complete a separate tank form for each tank. 4. A Current financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Last financial responsibility certification was submitted on 12/23/09 and the next one was due by 12/23/10. Complete and submit a copy of the financial responsibility within 30 days of receiving this report. 5 & 6. Current approved copies of the monitoring and response plans were not found on site. The monitoring and response plans must be current and approved by the EHD. Complete and submit a copy of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An approved copy will be returned for your records. 7. The alarm log and a maintenance log were not found on site. These records shall be kept on site for at least 3 years . Immediately locate and maintain the alarm and maintenance log on site and submit copies to the EHD within 30 days of receiving this report. 15. The secondary containment test results (tested on 11/18/08) indicate a failure under dispenser containment (UDC)1/2, 3/4, 5/6, 7/8, 87 secondary line run #2, #3, #4 and secondary line run #1. All secondary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD and submit passing results to this office. 23a. A designated operator employed by Franzen -Hill (Steve Zwahlen) performing designated operator monthly inspections did not provide notification to the EHD within 30 days of the change. Steve Zwahlen also signed on designated operator monthly report prepared by Tony Murch and Terry Hudson. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying all the designated operators for this facility within 30 days of receiving this report. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES UNLL BE BILLED AT THE CURRENT HOURLY RATE (5122).
