The 91 turbine sump sensor was raised approximately 6 inches off the lowest point of the sump and <br />the 87 turbine sump sensor was located higher on the tank saddle just above the water line and not <br />located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able <br />to detect a leak at the earliest possible opportunity. Immediately relocate this sensor to the lowest point <br />of the 87 and 91 turbine sump. The service technician, Steve Zwahlen, removed all the liquid from the <br />87 and the 91 sump and relocated the sensor to detect a leak at the earliest opportunity. <br />36. The 91 turbine sump sensor was raised approximately 6 inches off the lowest point of the sump and <br />the 87 turbine sump sensor was located higher on the tank saddle just above the liquid and not located <br />to detect a leak at the earliest opportunity. Steve Zwahlen, technician that performed testing on <br />12/14/10, informed that he had left the sensors at the,lowest point in the sumps to detect leak at the <br />earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br />possible opportunity. Immediately relocate this sensor to the lowest point of the sump. The service <br />technician, Steve Zwahlen, removed all the liquid from the 87 and the 91 sump and relocated the sensor <br />to detect a leak at the earliest opportunity. <br />21. Liquid was observed in the 87 turbine sump (approximately 1 gallon) and 91 turbine sump (14 <br />gallons). If water could enter into the secondary containment by precipitation or infiltration, it must be <br />removed and disposed of properly. Immediately remove this liquid, make a hazardous waste <br />determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the 87 <br />and 91 turbine sump is maintained free of liquid. The service technician, Steve Zwahlen, removed all <br />the liquid from the 87 and the 91 sump and relocated the sensor to detect a leak at the earliest <br />opportunity.
