w db <br />11.0 MEDICAL SURVEILLANCE PROGRAM <br />A medical surveillance program will provides a means of <br />selection of employees who are physically able to safely <br />perform the work assigned and by monitoring their health on a <br />regular basis. The program consists of a pre-employment <br />evaluation to determine fitness for the job assignment, an <br />annual evaluation based on length of assignment, and an end <br />of employment evaluation, made available as specified by the <br />Health and Safety Officer. In addition, a special evaluation <br />is warranted upon notification by the employee that they have <br />developed signs or symptoms indicating a possible over <br />exposure to hazardous substances. <br />Medical monitoring will be implemented for all site <br />personnel. All staff personnel included in an annual medical <br />monitoring program will have had an examination which <br />includes: <br />Medical and Occupational History <br />Physical Examination <br />Pulmonary Function Test <br />Six Frequency Audiogram <br />Urinalysis, with microscopic <br />Complete Blood Count <br />SMA 12 Chemistry Screen <br />Chest X -Ray <br />Drug Screen <br />morphology and dipstick <br />All personnel employed specifically for this project will <br />receive an equivalent physical examination. <br />All visitors and regulatory personnel who will enter the <br />exclusion area are expected to demonstrate participation in a <br />medical program which is equivalent to or exceeds this <br />program. <br />
