All soils must remain on-site until it is determined that weather the soil is <br />considered hazardous waste or not. The analytical results for the classification <br />of the soil must be submitted to PHS-EHD. The destination of the soil and <br />the shipper must have prior approval by PHS-EHD. <br />2. The tank tracking records are incomplete.Properly • •lete tank tracking <br />records and b <br />3. The backfill excavation certification is incomplete. Complete and submit to <br />PHS-EHD. <br />Installation Plan: <br />1. The plan indicates that the routes of the piping will be determined in the field <br />and that the plan is only a schematic. The contractor must provide a detailed <br />"as built" drawing prior to the final inspection. <br />2. The drawing of the tank is not clear as to the type or construction of the over- <br />spill containment. Provide type and product information sheet. <br />3. The drawings do not indicate that the dispenser containment will be equipped <br />with continuous monitoring. Provide information on type of continuous <br />monitoring to be used. <br />4. The drawings do not indicate that the tank will be equipped with over -fill <br />protection.
