Complete and submit a copy of a UST Installation Certificate of Compliance <br />(Former Form C) to this office no later than 30 days after work is completed. <br />5. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />6. The technician must print out a copy of the alarm history prior to performing the <br />"cold start" of the Veeder Root system and have the copy available at the time of <br />EHD inspection for review. <br />7. Third party tank integrity test and a line test must be performed upon completion <br />of work, if concrete is broken, prior to the contractor signing off of the jobsite. <br />The test results must be submitted to SJCEHD within 7 days of completing this <br />project. <br />8. An approved test must be performed to determine the integrity of the under <br />containment dispenser pan. <br />9. All product installed must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />California Code of Regulations, Title 23, Chapter 16 Table B for complete list.). <br />Pursuant to the Underwriters Laboratories Inc (UL) listings, only those products <br />bearing the UL mark will be considered listed. Any part/product not marked with <br />the UL label will be rejected. <br />10. Permit expires 90 days from date of approval.
