The designated operator failed to check if maintenance was performed for the August 26, 2011 and <br />June 3, 2011 L4 annular space sensor alarm on the designated operator monthly inspection reports. <br />During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure <br />that the designated operator performing monthly inspection are including the required information on the <br />reports. Submit a written explanation what had occurred on August 26, 2011 and June 3, 2011 and any <br />responsive action taken for the alarms. <br />205. The September 2011 and January 2012 designated operator monthly reports were not found on <br />site. Designated operator monthly inspection reports for the previous 12 months shall be retained on <br />site. Locate and ensure copies of the previous 12 months of the designated operator monthly inspection <br />reports are maintained on site. Submit copies of September 2011 and January 2012 reports to EHD. <br />- Found September 2011 monthly report- misfiled. Unable to find January 2012 monthly report. <br />206. The designated operator employee training for Sunny Singh, "Jasper", Sandra Perkins and Jagjit <br />Singh was last conducted May 25, 2011. The next annual training was due by May 2012. The <br />designated operator shall train facility employees for which he or she is responsible in the proper <br />operation and maintenance of the UST system once every 12 months. The training shall include, but is <br />not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4.
