Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility to the EHD within 30 days of receiving this report. <br />105. UST facility form is lacking BOE Maintenance Fee Account Numbers or BOE number is incorrect. <br />Immediately complete and submit a copy of the facility form to the EHD. <br />106. Secondary containment testing was last performed on May 28, 2009 and was due by May 28, <br />2012. Testing has not been completed and is one month past due. Secondary containment testing is <br />required once every 36 months. Immediately schedule this test and provide 48 hours notification to the <br />EHD. <br />109, 110, & 111. Annual monitoring system certification, leak detector testing, and spill container testing <br />were last performed on May 23, 2011 and was due by May 23, 2012. Testing has not been completed <br />and is one months past due. These tests are required once every 12 months. Immediately schedule <br />these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection <br />will be charged at our current hourly rate. <br />201. Maintenance invoices and alarm log were not found on site. These records shall be kept on site for <br />at least 3 years. Immediately locate and maintain the maintenance record and alarm log on site and <br />submit copies to the EHD within 30 days of receiving this report. <br />204. The designated operator failed to check if maintenance was performed for the August 26 and June <br />3, 2011 alarm on the designated operator monthly inspection reports. During the monthly inspection, the <br />designated operator shall check that all required testing and maintenance for the UST system have been <br />completed and document the dates they were done.
