Facility lack new tank forms. Any change of information on these forms must be submitted to the <br />EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank <br />forms to the EHD. Complete a separate tank form for each tank. <br />6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans <br />shall be retained on site at all times. Immediately locate and retain a copy on site. <br />23c. The January, 2011, designated operator monthly inspection report was not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br />reports are maintained on site. Submit copies to the EHD within 30 days of receiving this report. The <br />DO for the facility was on site at the time of inspection and provided a reprint of the January, 2011, <br />report. <br />23d. The designated operator employee training for Jagi and Sandi, hired in November, 2010, was not <br />performed. The designated operator shall train facility employees for which he or she is responsible in <br />the proper operation and maintenance of the UST system once every 12 months. The training shall <br />include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD within 30 days of receiving this report. <br />Notes <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, within 30 days of receiving this report. <br />Provided blank tank forms to be fill out by the facility and submitted to this office <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: <br />Title: <br />EHD 23-02-003 Rev 08/10/10 I CONTINUATION FORM <br />
