Olive Ave., Stockton CA <br /> The San Joaquin County Environmental Health Department(EHD)has completed the review of the application for <br /> permit(SR0051842)to install a spill bucket on the 91-underground storage tank(UST)at the above noted address. <br /> The permit has been approved with the following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department and the San Joaquin Valley Air District for any applicable permits and <br /> requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. All components must be certified by an independent testing organization and labeled as required by the <br /> organization.(UL,ULC,ANSI,API,ASTM,etc. See California Code of Regulations(CCR),Title 23 <br /> Chapter 16 Table B for complete list). The component must bear the appropriate mark as required by the <br /> independent testing organization. <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid,current contractor's licenses issued by the Contractors State <br /> License Board(may be issued to the installation company),and <br /> b. All valid,current California UST certificates issued by the International Code Council(ICC),that <br /> are applicable to the work being performed,and <br /> c. A valid,current certificate of training from the manufacturer(s)of the component(s)being tested, <br /> repaired,or serviced. <br /> 5. Each person installing or testing any component must hold a certificate of training from the manufacturer <br /> of that component. <br /> 6. Contact Ray von Flue(468-9848)to arrange the following inspections appointments(a 48-hour notice is <br /> required for inspection appointments): <br /> a. Verification of five-gallon capacity of the premium spill bucket. <br /> b. Drain valve operation <br /> c. Sixty minute or equivalent test on spill bucket <br /> 7. If concrete is broken near the UST's,a third party tank integrity test(or equivalent) must be performed <br /> once concrete is poured in accordance with the manufacturer's guidelines.Results must be received by <br /> EHD within 7 days of returning the site to operating condition. A UST Install/Retrofitting ICC Cert.will <br /> also be required. <br /> 8. A"Certificate of Compliance for Underground Storage Tank Installation Form C" shall be submitted by the <br /> owner or the owners'agent[pursuant to(CCR)2635(d)]before this upgrade will be considered complete <br /> by EHD. <br />
