The Bravo box float and chain in the sump failed to stop the flow of product at the dispenser r #1, <br />#3/4, and #9/10 when tested. <br />-- All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow <br />of product at the dispenser when it detects a leak. <br />* The service technician adjusted the chain and verified functionality. Corrected on site. <br />18. The diesel direct -bury spill container failed when tested. Inventory read 9922 gallons. <br />-- All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill <br />or overfill until it is detected or cleaned up. <br />* Immediately discontinue deposition of petroleum into this tank until the component is repaired or <br />replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br />If the spill container can't be replaced immediately, there is a possibility that the UST system may be <br />red tagged to prevent fuel inputs. <br />NOTE <br />The 3 annular tank sensors passed functionality test, but were all in a state of disrepair with cracked bell <br />housing. Submit a permit application to EHD by a licensed and certified contractor to replace the <br />annular sensors to EHD by January 3, 2011. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EH InspeaptReceiv <br />d By: <br />Zed <br />Title: <br />U w 1001, <br />EHD 23-02-003 Lo0/10 CONTINUATION FORM <br />
