2007 <br />The HSM has the primary responsibility for ensuring that the Policies and Procedures of this <br />HASP are implemented. The HSM ensures that all personnel designated to work at the site are <br />qualified according to Medical Surveillance and Health and Safety Training consistent with <br />OSHA's 29 CFR 1910.120 Regulations covering Hazardous Waste Operations and Emergency <br />Response. The HSM is responsible for providing the appropriate monitoring and safety <br />equipment necessary for the implementation of this HASP. Any significant changes to the <br />HASP must be approved by the HSM. The HSM is Richard Arbuckle <br />r •; A J 014• <br />The SSHO, Lead TES Employee on-site, has the responsibility to implement this HASP. The <br />SSHO will investigate all accidents and incidents occurring at the site. The SSHO will conduct <br />safety briefings and site-specific training for on-site personnel. The SSHO will accompany all <br />OSHA and other Governmental Agency Personnel visiting the site in response to health and <br />safety issues. He is responsible for modifying and/or developing new procedures, after <br />consultation with the HSM, when site or environmental conditions change due to natural <br />causes or the site operation. <br />The SSHO has Stop -Work Authorization if they determine that an imminent safety hazard or <br />potentially dangerous situation exists. Authorization to Proceed with Work will be verified by <br />the HSM. The SSHO will be responsible for implementing evacuation procedures, including the <br />shutting down of appropriate equipment, removing equipment from areas, and coordinating <br />necessary emergency services on-site. <br />No work is to be performed without the SSHO or HSM on site, unless an alternate <br />SSHO or HSM has been approved in advance by (the project coordinator) of Tait <br />Environmental <br />All team members, including all employees of subcontractors working at the site, will be <br />responsible for understanding and complying with all site Health and Safety Requirements. All <br />team members will have been provided with any site-specific training regarding the hazards <br />involved for this particular project. Subcontractors will be responsible to provide their <br />employees with appropriate equipment and clothing. <br />Interface of trades involved with construction will occur. Communication between the trades <br />will exist to point out the site-specific potential physical and/or chemical hazards. <br />11/07/2005 revision 5 3 <br />
