This service request is in response to the failed secondary containment test on <br />October 25, 2010. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. <br />b. 24-hour hydrostatic test (or equivalent) on the piping sump. <br />c. Secondary containment testing on vent line. <br />Please be aware that the facility cannot operate the UST tank prior to the EHD <br />inspection and approval. <br />4. If concrete will be broken near the UST or piping, a secondary containment test must be <br />performed on the UST or piping once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />s J <br />1 � <br />
