A copy of these plans could not be <br />located on site at time of inspection. Obtain a copy of these documents and retain on site at all times. <br />Correct immediately. <br />The monitoring plan was updated and sent to San Joaquin County, Environmental Health Department <br />(EHD) on 7/31/06 by RHL Design Group; a copy was posted on-site on 8/10/06. <br />This office does not have a current statement from UST owner that identifies current designated operator <br />(DO). A George Koffel has been performing D. 0. inspections, and is not listed on the D. 0. agreement <br />statement currently on file with this office. Update this document and submit a copy to this office by <br />8/2/06. <br />The updated DO statement was submitted to San Joaquin County, EHD, by Tait Environmental Systems on <br />7/31/06; a copy was also posted on site. <br />Spill container failed annual leak test. Have this spill container repaired or replaced under a permit from <br />this office immediately. <br />The repair permit was obtained on 7/28/06 from San Joaquin County, EHD; the repairs were completed on <br />8/3/06 with passing results. <br />Please see attached Return to Compliance Certification.
