Repair or replace & obtain permit to replace from this office & repair or replace by 9-4-05. Contact this office at least 48 hours in advance to witness testing. Repairs completed; County not required to be witness at testing of repairs per Willie Ng on 8/11/05 telephone call with Gary Nygren of Tait Environmental System. • Monitoring & response plans are outdated (2001). Update the monitoring and response plans and submit a copy to this office by 9/4/05. Completed and submitted by RHL Design Group on 8/10/05. • Employees are required to be trained by the designated operator in the proper operation and maintenance of the UST system. At least one employee trained by the D.O. is required to be onsite during normal business hours. Have D. 0. train immediately and submit a signed copy of certification of training to this office by 9/4/05. Designated operator training form supplied by TAIT documenting Calvin Muir's training completed on 8/15/05.
