Once is has been determined that the emergency no longer exists, the emergency <br /> coordinator must provide for the treating, storage or the disposing of all recovered <br /> waste, contaminated soil or debris or surface water form the release or emergency, <br /> and ensure that all emergency equipment is cleaned and ready for its intended use, <br /> before operations at the facility or plant are resumed. Within 15 days of the <br /> emergency, the facility or the owner,or operator of a treatment, storage or disposal <br /> facility shall submit a written report to the Regional Administrator in the EPA region <br /> where the emergency took place... and provide the following information. <br /> *The name, address and telephone number of the generator, owner or operator. <br /> *The name, address and telephone number for the facility, plant or location, if <br /> different than that of the generator, owner or operator. <br /> *The date, time and type of incident. <br /> *The name and quantity of the materials involved. <br /> *The extent of any injuries or fatalities, if any. <br /> * An assessment of the actual potential hazards to human health or the environment, <br /> if applicable. <br /> *And the estimated quantity and disposition of recovered materials that resulted form <br /> the incident. <br /> Page 14 <br />
