Qualified service technicians shall possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested. <br /> d. Each individual installing or testing any component shall hold a certificate <br /> of training from the manufacturer of that component. <br /> 7. Tank integrity tests must be performed upon completion of work, if concrete is <br /> broken,prior to the contractor signing off of the jobsite. The test results must be <br /> submitted to SJCEHD within 7 days of completing this project <br /> 8. Complete and submit a copy of Underground Storage Tank Certification of <br /> Installation/Modification(Former Form C) to this office no later than 30 days <br /> after work is completed. <br /> 9. Permit expires 180 days from date of approval.
