The 1-29-2016 monitoring <br /> system certification and spill container testing report was not found onsite at the time of inspection. These records <br /> shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit <br /> a copy of the mossing testing report to the EHD. <br /> This is a repeat violation, Class 11. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> The following was noted: <br /> 1) The diesel underground fiberglass fitting located adjacent to UDC 3/4 was replaced under permit number <br /> SR0072496, this repair was required to be reinspected/retested 6-month upon installation, however this testing was <br /> not conducted. Immediately schedule with EHD all required follow up testing in connection with this SR0072496 <br /> permit. <br /> 2) The Veeder Root system appears to have been upgraded in September 2016 and a permit was not obtained <br /> from the EHD. According to the designated operator inspection report dated 9-6-2016, the system is "missing alarm <br /> history due ISD upgrade". Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. Immediately obtain a permit from the EHD for this repair. A"Failure to Obtain a <br /> Permit" penalty applies. Submit proof of correction to the EHD. <br /> This is a Class 11 violation. <br /> 301 HSC 25292.1 UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Monthly overfill alarms (for the T1 UST tank) have been noted from February 2016 through March 2017. The <br /> underground storage tank system shall be operated to prevent unauthorized releases and overfills. Ensure that the <br /> T1 tank system is not being routinely overfilled and that the the required overfill prevention is being maintained. <br /> Take all necessary precautions to prevent this in the future. Submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Onsite for a routine UST (paperwork) inspection, the routine monitoring system certification was due by <br /> 1-29-2017 and is past due at this time. Immediately schedule the monitoring system certification inspection, <br /> which will be a billable re-inspection billed at the EHD's current hourly rate. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> FA0000421 PR0231876 SCO01 03/09/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
