The following is an itemized list of underground storage tank violations that have not been addressed for 76 MINI MART as of June 07, 2016. Open violations from January 29, 2016 inspection Violation #207 - Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment testing on all 2015 designated operator monthly inspection reports. According to the received secondary containment testing results, the secondary containment testing was conducted on 3-2-2015; the designated operator reports list it as 3-18-2015. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Violation #209 - Designated operator employee training not performed or log not kept. 1) The designated operator employee training log doesn't contain all the required information, the employee hire dates were not provided for Mr. Mangir Singh and for Mr. Hartaj Singh. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that all employees have been trained by the designated operator, maintain the list on site, and submit a copy of the complete training records to the EHD. 2) According the the February 18, 2015 routine underground storage tank inspection, no employee training was conducted for this facility. The employee training log was observed at the time of the most current inspection. Open violations from February 18, 2015 inspection Violation #204 - Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site.
