Designated operator monthly inspection reports for the previous twelve <br />months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated <br />operator monthly inspection reports are maintained on site. This DO report was observed onsite at the time of <br />inspection. This was corrected on site. <br />his is a repeat violation. Class II. <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />1) The designated operator employee train. .,; log doesn't contain all th squired information, the employee hire <br />dates were not provided for Mr, Mangir Singh and for Mr. Hartaj Singh. The designated operator shall train facility <br />employees for which he or she is responsible in the proper operation and maintenance of the UST system once <br />every 12 months. The training shall include, but is not limited to: <br />1. Operation of the LIST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that all employees have been trained by the designated operator, maintain the list on site. and submit a copy <br />of the complete training records to the EHD. <br />2) According the the February 18, 2018 routine underground storage tank inspection, no employee training was <br />conducted for this facility. The employee training log was observed at the time of the most current inspection. <br />This is a repeat violation, Class ll. <br />CCR 2636(%2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 91 VMI line leak detector tailec Detect a ieak when tested. rail line leak detectors shall be capable of <br />detecting a 3 -gallon per hour leak at "i,3 psi. The service technician adjusted the leak detector retested it, and it <br />sf'�assed.
