This information was required to be submitted by February 18, 2014. <br />The conducted monitoring system certification was incomplete, the diesel line leak detector could <br />not be tested at the time of inspection. Immediately schedule and conduct this testing. The follow <br />up inspection concerning this matter, will be billed at the current hourly rate ($125). <br />Aote: All EHD staff time associated with failing to comply, including the issuance of this letter, <br />billed at the current hourly rate ($126). Failure to submit the completed information immediate <br />-nay result in a re -inspection, additional violations, and/or further legal action. <br />Items requiring immediate correction include: <br />1) Return to Compliance Certification (attached hereto) <br />2) Corrective action statement describing how each of the following violations was addressed <br />3) Violation 109: Completion of monitoring system certification <br />44) Violation 105: UST facility operating permit application and UST tank information are not current <br />5} Violation 112/ The diesel UST is not failsafe and doesn't meet the requirements ofannual line <br />tightness testing <br />6) Violation 201: No maintenance or repair records were found onsite at the time of inspection <br />7) Violation 204: The designated operator for this facility failed to inspected all of the required areas <br />8) Violation 204: Designated operator failed to check and note correct test dates <br />9) Violation 303: Bravo box in the UDC % is not functional, correct immediately under permit and <br />
