The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />Contact Elena Manzo at (209) 953-7699 to arrange the following appointments (a 48-hour notice is <br />required for inspection appointments). <br />• Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br />monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br />and the Health & Safety Code. If it is determined that a cold start is necessary, a copy of the <br />alarm history shall be printed prior to performing the "cold start" of the monitoring system and <br />made available at the time of EHD inspection. <br />Please be aware that the facility cannot operate the UST system prior to the EHD inspection <br />and approval. <br />4. If concrete will be broken near the tank, a secondary containment test must be performed on the <br />tank once concrete is poured in accordance with the manufacturer's guidelines. <br />
