UST Monitoring Plan <br />2. Incident Response Plan <br />3. Tank, line and leak detector test dated 5/18/98. Note that the waste oil tank was not required <br />to be tested at this time. <br />4. Monitoring certification. Note that there is no interstitial probe on the waste oil tank. Banks <br />& Company will be obtaining the necessary permits and installing the interstitial probe in the <br />very near future. The probe will be connected to the TLS -350 monitor. This should resolve <br />the issue of a conditional permit. <br />5. Maintenance repair record for the moisture found underneath dispenser #1 & #2. <br />6. Additional maintenance logs that were called into our central dispatch. Please note that it is <br />a Dealer requirement to maintain written records of all maintenance and inspections of the <br />UST system. Please contact the Dealer, Huyenthanh Thi Phan, at (209) 835-5358 for <br />additional records. <br />7. A and B forms. <br />Under Tosco ownership, there have been no unauthorized releases at the subject facility. <br />Furthermore, Tosco is unable to locate the permit for replacement of the leak detector, nor do we <br />know when it was replaced.
