The <br />permit has been approved with the following conditions: <br />1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control District for <br />applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />3. At least one person who holds the International Code Council "UST Installer/Retrofitter" <br />certification must be onsite AT ALL TIMES when work is performed. <br />4. A "Certificate of Compliance for Underground Storage Tank Installation Form C" shall be <br />submitted by the owner or the owners' agent [pursuant to California Code of Regulation (CCR) <br />2635(d)] before this upgrade will be considered complete by SJCEHD. <br />5. Contact Steven C. Shih at (209) 468-9850 to arrange the following inspection appointments; a 48- <br />hour notice for inspection appointments is required. <br />a. Corrosion protection: prior to backfilling around the spill buckets, corrosion protection of <br />the risers must be verified. <br />b. Overfill prevention: overfill prevention system in compliance with CCR 2635 (b)(2) must be <br />verified. <br />c. Spill Bucket: after concrete was poured, spill bucket with minimum of 5 gallon capacity and <br />a 30 -minute lake test in compliance with CCR2635(b)(1) must be verified. <br />d. Third party tank integrity tests must be performed once concrete is poured, prior to the <br />contractor signing off of the jobsite. The test results must be submitted to SJCEHD within 7 <br />days after the test. <br />6. Permit expires 90 days from the approval date. Note: if an extension to the expiration date <br />is required, a written request must be received by this office prior to the expiration date. <br />Thank you, <br />Steven C.
