Included is the <br />Repair Application, Fee Worksheet & Service Request, Specification Sheets and Application fee <br />of $279.00, Tait check # 14828. <br />The scope of work includes: <br />• Install (4) electrical penetrations inside dispenser pan #3/4 and #7/8. <br />• Retest the dispenser pans per Senate Bill 989 <br />Penetrations to be used are NOT split boots. No concrete will be broken at this time.
