According to the maintenance records on site, a cold start of the monitoring panel was conducted on October 29, 2013 and a permit was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the 10/29/13 cold start. A "Failure to Obtain a Permit" penalty applies. This is a Class II violation. 201 Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. Records of all alarms and the response to these alarms was not found on site. The site is keeping alarm logs, but these logs are incomplete and do not list all of the alarms for the site. The alarm logs for November 2013 through September 2014 only included sensor out alarms on November 14, 15, 16, 17, 19, and 21, and do not state which sensor was in alarm. The other days on the alarm logs are either blank or "all functions normal'. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 204 Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately update the monitoring plans in CERS and retain a current, approved copy on site. This is a repeat violation, Class II.
