The permit has been approved with the following conditions: Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable permits and requirements. 2. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing work. All new addenda supersede the previous ones. 3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments (a 48-hour notice is required for inspection appointments). Please be aware that inspections for emergency work must be scheduled within five days of permit approval. a. Sensor functionality: Functionality of the monitoring system must be verified to ensure that monitoring has been restored to meet the requirements of Title 23 California Code of Regulations and the Health & Safety Code. b. A copy of the alarm history shall be printed prior to performing the "cold start" of the Veeder Root system and made available at the time of EHD inspection. Please be aware that the facility cannot operate the UST system prior to the EHD inspection and approval. 4. Qualified service technicians must possess or work under the direct and personal supervision of an individual physically present at the work site who possesses all of the following: a. One of several permissible valid, current contractor's licenses issued by.the Contractors State License Board (may be issued to the installation company), or a Tank Tester's license issued to the individual by the State Water Board, and b. A valid, current California UST Service Technician certificate issued by the International Code Council (ICC), and c. A valid, current certificate of training from the manufacturer(s) of the component(s) being tested, repaired, or serviced.
