Save Mart Supermarkets is the property owner and is <br /> responsible for the UST system. <br /> VIOLATIONS/OBSERVATIONS: <br /> - SJC EHD has not received: Permit operating fees for 2010 & 2011 year, Operating Permit application <br /> (Facility and Tank information)- HSC 25284, CCR 2711. <br /> - UST is overdue for the annual Monitoring System Certification, Spill Bucket and Leak Detector testing (last <br /> performed 08/31/2009) - CCR 2638., HSC 25284.2, HSC 25291(f). <br /> - Facility has been notified by the State Water Resource Control Board on 06/20/2007 to perform an <br /> Enhanced Leak Detection (ELD)testing - CCR 2640(e). <br /> - UST is not continuously monitored, site is unmanned- CCR 2636(f). <br /> CORRECTIVE ACTIONS/ORDER: <br /> - Submit current Operating Permit Application (Facility and Tank Information documents provided). <br /> -Schedule the annual Monitoring System Certification, Spill Bucket, Leak Detector testing and the ELD <br /> testing.
