Item 7. The Franchisee was reminded that all alarms(except low fuel levels and <br /> Healy System alarms)should be recorded on the Daily Monitoring System <br /> Inspection Log, as well as called in to Quik Stop's Fuel Department for <br /> maintenance dispatch. <br /> 2. Item 23c. A copy of the missing July 2011 DO monthly inspection checklist is <br /> attached. The DO could provide no explanation as to why a copy could not be <br /> found on site. A copy will be sent to the facility. <br /> 3. Item 23c. The July 3,2011 L3 Fuel Alarm(91 STP Sump)was,in fact, <br /> documented in the July 2011 Designated Operator Monthly Inspection Checklist. <br /> The alarm was called in to QS Maintenance,recorded,and responded to by Elite <br /> IV Contractors(see attached Repair Log,and invoice describing activities <br /> performed). <br /> Return to Compliance Certification is also attached. <br />
