D. Number 003934 <br /> Dear Ms. Campbell: <br /> Enclosed is a check in the amount of$1190.00 for payment of the attached invoice#008732 <br /> dated 03/11/94 which is for the 1994 permit fees for seven regulated underground storage <br /> tanks (USTs)for the period January 1, 1994 through December 31, 1994. The invoice, <br /> postmarked 3/18/94 was received by W.W. Schwartz on March 28, 1994 and is due by <br /> May 11, 1994. All of these USTs are located at the Lawrence Livermore National Laboratory's <br /> Site 300 Facility. The seven USTs are: 829-DlUl (0031),834-DlU1 (0012),865-D1 U1 (0022), <br /> 871-DlUl (0017),875-D1U2 (0032),879-D1U1 (0001),and 882-DlU1 (0029). <br /> A list titled"1994 UST PERMITS / SITE 300"that gives details about the seven regulated USTs is <br /> attached. These seven USTs were also permitted in 1993 in addition to eight others. The status <br /> of the eight other USTs that were permitted in 1993 follows. Two USTs, 814-DlU1 (#0026)and <br /> 830-DlU1 (#0006)were closed during 1993. Four USTs,827-DlUl (#0008),836-DlU1 (#0027), <br /> 879-GlUl (#0019),and 879-G2U1 (#0018)were closed in the first quarter of 1994. Two USTs, <br /> 805-DlUl (#0025)and 827-D2U1 (#0007) had closure plans submitted to your office and are <br /> planned to be closed in 1994. <br /> Please note that some presently permitted USTs may be removed from service and closed or <br /> are currently proceeding through the tank closure process.
