Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />RE: Revised approval letter for Lawrence Livermore National Laboratory — Site 300 <br />Your permit application to retrofit an underground storage system (UST) at building 882 ,building 875 and <br />building 871 at the above referenced location has been approved with the following conditions. <br />Removal Conditions: <br />1. Contact the local fire department for their permit requirements. <br />2. Samples must be taken and analyzed for TPH-D and BTEX at building 882 and 871, and the state <br />certified lab that will run the analytical tests shall be named prior to any work performed. <br />3. "Authorization to release" form shall be completed and submit to this office prior to schedule soil <br />sampling. <br />4. EHD must witness the soil sampling. Contact EHD to arrange this inspection appointment. <br />5. The nearby storm drains shall be protected from any possible contamination. <br />Installation Conditions: <br />1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for applicable <br />permits. <br />2. Any changes in information or scope of work must be submitted to EHD prior to commencing work. <br />3. Submit certificate of training issued by the tank and piping manufacturers prior to commencing <br />work. <br />4. All piping and connections shall bear an UL label. <br />5. All piping must slope to a containment sump with an audible and visual alarm. <br />6. All equipment in contact with backfill must have corrosion protection. <br />7. Overfill prevention must satisfy CCR 2635(b)(2). <br />8. If automatic pump shut down is provided it must satisfy 2636(g) of California Code of Regulations <br />(CCR). <br />9. Contact EHD for the following inspections: <br />a. Sampling. <br />b. Primary piping and Secondary Piping. <br />c. Piping slope. <br />d. Final inspection: <br />i. A final inspection must be performed prior to returning this station to operation. <br />ii. A third party certified tank integrity test must be performed for building 882 and <br />results received prior to scheduling the final inspection. <br />Page I of 2 <br />
