(DCM) respectfully request the expeditious processing of a Permit <br />to Repair for SB 989 Testing previously completed, the required repairs needed are at Bldgs. 871, 875, 882 <br />Lawrence Livermore National Laboratory — Site 300 (LLNL) in Livermore California. <br />DCM will be providing all labor, equipment and materials for the purpose of making the fueling systems <br />at the three respective sites in compliance with the new SB 989 regulations at the respective sites. <br />Attached are 3 copies of the: 1. Permit Form <br />2. Fee Sheet <br />3. Statement/Scope of Work for each site. <br />4. Site Map <br />5. Drawings/locations of each site. <br />6.
