COORDINATORS <br />Carl Borgman, REHS <br />Mike Huggins, RENS, RDI <br />Margaret Lagorio, REHS <br />Robert McClellon, RENIS <br />Jeff Carruesco, RENS, RDI <br />Kasey Foley, REHS <br />The San Joaquin County Environmental Health Department (EHD) has completed the <br />review of the application for permit to: <br />install a new ECPU main board in the TLS 350 and perform a cold start on the system <br />-install a new cap on the unleaded (87) direct bury spill bucket and perform a 60 minute <br />hydrostatic lake test or equivalent <br />The application has been approved with the following conditions: <br />1. Contact the local fire department Local Planning or Building Department, and the <br />San Joaquin Valley Air Pollution Control District for any applicable permits and <br />requirements. <br />2. Contact Michelle Henry at (209) 953®7699 to arrange the following inspection <br />appointment (a 48-hour notice is required for inspection appointments): <br />a. Functionality test to ensure that monitoring has been restored to meet the <br />requirements of California Code of Regulations Title 23 and the Health & <br />Safety Code. (Verified on September 8, 2008) <br />b. Verify that the spill bucket holds a minimum of 5 gallons and pass a <br />60 minute lake test or equivalent after concrete is poured. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all <br />of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, <br />
