Box 2009 <br /> Stockton, CA 95201 <br /> SUBJECT: Application for Permit to Close the 875-31R Underground Storage <br /> Tank at Lawrence Livermore National Laboratory's (LLNL) Site 300 <br /> Dear Ms. Cotulla: <br /> Enclosed is a permit application for closure of the 875-31R underground <br /> storage tank at Lawrence Livermore National Laboratory's (LLNL) Site 300. The <br /> accompanying appendices provide additional information as follows: <br /> Appendix A: Plan View of Proposed Soil Sample Locations for the <br /> Closure of the 875-31R Underground Tank <br /> Appendix B: Supplemental Information <br /> Appendix C: Authorization to Release Analytical Results, <br /> Geotechnical Data and Site Assessment Information <br /> Appendix D: Fee Worksheet <br /> Appendix E: Underground Tank Disposition Tracking Record <br /> (to be completed at time of closure activities) <br /> Prior to commencement of closure activities the Plant Engineering Department <br /> will obtain a permit for tank closure from the LLNL Fire Department. This <br /> permit will be available for inspection by San Joaquin Local Health District <br /> field personnel during tank closure activities. <br /> The Soil Sample Map (Appendix A) and the Supplemental Information (Appendix B) <br /> describe a soil sampling plan developed to assess the type and degree of <br /> contamination that may be associated with the 875-31R tank. Although the tank <br /> was tested and found to be leak tight, there may have been instances when rain <br /> runoff washed over the concrete pad underlying the drum rack. This may have <br /> possibly spread residue from the pad to soil in a localized area around the <br /> tank. Locally contaminated soil, if found, will be removed and disposed of as <br /> hazardous waste at the time of closure.
