Livermore2110
Lawrence i I Laboratory
January 20, 1989LIA
"*
Ms. Laurie Cotulla `
San Joaquin Local Health District
Environmental Health Division
P.O. Box 2009
Stockton, CA 95201
SUBJECT: Application for Permit to Close the 875-31R Underground Storage
Tank at Lawrence Livermore National Laboratory's (LLNL) Site 300
Dear Ms. Cotulla:
Enclosed is a permit application for closure of the 875-31R underground
storage tank at Lawrence Livermore National Laboratory's (LLNL) Site 300. The
accompanying appendices provide additional information as follows:
Appendix A: Plan View of Proposed Soil Sample Locations for the
Closure of the 875-31R Underground Tank
Appendix B: Supplemental Information
Appendix C: Authorization to Release Analytical Results,
Geotechnical Data and Site Assessment Information
Appendix D: Fee Worksheet
Appendix E: Underground Tank Disposition Tracking Record
(to be completed at time of closure activities)
Prior to commencement of closure activities the Plant Engineering Department
will obtain a permit for tank closure from the LLNL Fire Department. This
permit will be available for inspection by San Joaquin Local Health District
field personnel during tank closure activities.
The Soil Sample Map (Appendix A) and the Supplemental Information (Appendix B)
describe a soil sampling plan developed to assess the type and degree of
contamination that may be associated with the 875-31R tank. Although the tank
was tested and found to be leak tight, there may have been instances when rain
runoff washed over the concrete pad underlying the drum rack. This may have
possibly spread residue from the pad to soil in a localized area around the
tank. Locally contaminated soil, if found, will be removed and disposed of as
hazardous waste at the time of closure. Any wide spread contamination found
to be present will be addressed under the Remedial Investigation/Feasibility
Study at the B-815 and General Services Area (Task 6 in the Site 300
Environmental Restoration Work Plan; July 13, 1988; UCAR-10247) .
The EPA Site Number for this site is CA2890090002.
Enclosed is a check for $120.00 to cover the fees associated with the plan
review and permanent closure of one tank (see Appendix D) .
`y
c
An Equal Opportunity Employer•Universityof California•P.O Box 808 Livermore,Califomia 94550•Telephone(415)422-1100•Twx 910-386-8339 UCLLL LVMR
