The designated operator <br /> addressed the 1_6:"92" sump STP sump fuel alarms on the designated operator monthly inspection reports for: <br /> 7/25/16. 10/24/16, 11122/16, 1/25117, 2124/17, 3128/17 and 4124/17 : however, the designated operator failed to <br /> check the box"Yes"for Iters 4 "Has each alarm for the previous month been responded to appropriately?" Instead <br /> the designated operator checked"NA". <br /> During the monthly inspection, the designated operator shallreview the alarm history for the I;revioas month, checE; <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Instead of entering 5/7/2015, the <br /> designated operator entered 5/7/2016. During the monthly inspection, the designated operator shall check that all <br /> required testing and rnaintc:..rrce for the UST system have been conipleted and document the dates they were <br /> done.
