Hammer Ln, Stockton, CA 95209 SR0070949 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the Amended application for a permit to replace four existing Gilbarco Advantage dispensers <br />and perform a cold start on the underground on the underground storage tank (UST) system at <br />the above noted address. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Fatinah Zareef at (209) 953-7311 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Visual inspection: Verify that all potential fuel leaks in the dispenser will flow to the <br />under dispenser containment. <br />b. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that the monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />(may be issued to the installation company), or a Tank Tester's license issued to the <br />individual by the State Water Board, and <br />
