The annual monitoring system certification was last completed on January 6, 2015 and the <br /> designated operator inadvertently entered January 14, 2015 in the monthly reports. During the monthly inspection, <br /> the designated operator shall check that all required testing and maintenance for the UST system have been <br /> completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 314 CCR 2631(a) Primary containment not tight(pre-Jul 2003). <br /> Approximately one-half cup of liquid was found in the diesel under dispenser containment, indicating a leak in the <br /> primary piping. All primary containment for the UST system must be tight. Immediately have a properly licensed, <br /> trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. A <br /> permit will be required if an UST component is replaced below the sheer valve. The liquid was removed at the time <br /> of the inspection. This was corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> David Myers, environmental contact, gave permission to perform the inspection, take photos and collect <br /> samples, if needed. <br /> An inspection report was provided to the facility foreman,Anthony Walter, and Jesse Berumen, designated <br /> operator, on the day of inspection. The report was amended by the Environmental Health Department (EHD). <br /> The amended report replaces the initial report. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the amended inspection report. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
