Maintenance and monitoring records for the last three years were not found on site. The facility alarm log failed to document and address the L5 Diesel Disp UDC Fuel Alarms on 5/26/17 and 9/14/17, and documented, but failed of address the L5 Diesel Disp UDC Sensor Out Alarm on 5/19/17. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the above-mentioned missing alarms on site and submit copies to the EHD. This is a repeat violation, Class Il. 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to document all the alarms from the attached alarm history on the June 12, 2017 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L5 Diesel Disp UDC Sensor Out Alarms on 5/19/17,8/30/17, and 9/5/17, and L5 Diesel Disp UDC Fuel Alarms on 5/22/17 and 5/26/17.Also, on the 10/9/17 monthly inspection report,the designated operator incorrectly wrote that the L6 sensor was replaced, instead of writing that the L5 sensor was replaced. During the monthly inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). Liquid was observed in the 87 and diesel under dispenser containments(UDCs). If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the 87 and diesel UDCs are maintained free of liquid. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 2/25/2018. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS.
