The facility alarm log failed to <br /> document and address the L5 Diesel Disp UDC Fuel Alarms on 5/26/17 and 9/14/17, and documented, but failed of <br /> address the L5 Diesel Disp UDC Sensor Out Alarm on 5/19/17. These records shall be maintained on site for at <br /> least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring <br /> equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis <br /> performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and <br /> maintain all maintenance and monitoring records for the above-mentioned missing alarms on site and submit copies <br /> to the EHD. <br /> This is a repeat violation, Class Il. <br /> 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the June 12, 2017 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: L5 Diesel Disp UDC Sensor Out Alarms on 5/19/17,8/30/17, and 9/5/17, and L5 Diesel <br /> Disp UDC Fuel Alarms on 5/22/17 and 5/26/17.Also, on the 10/9/17 monthly inspection report,the designated <br /> operator incorrectly wrote that the L6 sensor was replaced, instead of writing that the L5 sensor was replaced. <br /> During the monthly inspection,the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 87 and diesel under dispenser containments(UDCs). If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br /> accordingly. Ensure that the 87 and diesel UDCs are maintained free of liquid. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 2/25/2018. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Letter from Chief Financial Officer <br /> (if applicable), and the Owner Statement of Designated Operator Compliance.
