Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus to arrange the following inspections(a 48 hour notice is <br /> required): <br /> a. Verify corrosion protection on the fill riser and spill container before back <br /> filling. <br /> b. One hour hydrostatic test(or equivalent)on the spill container. <br /> c. Verify the five gallon capacity and drain valve operation of the spill container. <br /> d. Inspection of the drop tube to verify the 95%tank volume shutoff. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. A valid,current California UST Service Technician certificate issued by the <br /> International Code Council(ICC),and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required,a written request must be submitted to this office prior to the expiration <br /> date. <br /> Page 1 of 2 <br />
