The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Please beaware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Sensor functionality: Functionality ofthe monitoring system must beverified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code OfRegulations and the Health & Safety Code. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> @. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may beissued to the installation CO0pany\. oro <br /> Tank Tester's license issued tothe individual bythe State Water Board, and <br /> b. /\valid, current California UST Service Technician certificate issued by the <br /> International Code Council (|CC). and <br /> c. /\valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being taated, repaired, or serviced. <br />
