Victor Rd. Lodi, CA 95240 Program: UST SUMMARY OF VIOLATIONS CLASS I CLASS 0, or MINOR -Notice to Com Violations: # 2, 17, 23d. Repeat Violations: # 7, 21, 23c, 24. #2 This office received notification on 1-20-09 that secondary containment testing would be performed on 1-30-09. Copy of test results must be submitted to this office within 30 days of the test date. This office has not yet received a copy of the secondary containment test results. Correct immediately by submitting a copy of the secondary containment test results from the 1-30-09 testing to this office. #7 Maintenance records were not available for review for the following alarms: L4 91 annular low and high liquid alarms on 7-17-09; sensor out alarms for L1 91 stp sump, L2 89 stp sump, L3 91 stp sump on 8-19-09; sensor out, high, and low liquid alarms for L4 91 annular, L5 89 annular, and L6 87 annular on 8-19-09. Maintenance records for alarms must be maintained for 3 years. Correct immediately. Submit copies of maintenance records for these alarms to this office by 8-1-10. #17 The 89 VMI line leak detector failed the 3 gal/hr at 10psi leak test. Technician corrected this during the inspection by adjusting the 89 VMI line leak detector and retested it to pass. #21 Approximately 1 quart of water was in the 87 stp sump. Technician corrected this during the inspection by removing the water and placing it into the hazardous waste drum on site. #23c Designated operator (D.O.)reports from August 2009 through March 2010 list 2-24-09 as the date of the most recent monitoring system certification testing. The date should be 7-16-09. The D.O. has also listed date for the annual line test as 2-24-09 and 7-16-09. This is not accurate since positive shutdown on turbines has been verified. 87 line test was last done 2-4-09 and 87,89, & 91 line test were done 4-1-08. D.O. must completely and accurately complete the monthly D.O. report including documentation of test dates. Correct immediately by ensuring that the D.O. does this. #23d The two employees on site did not have documentation on site indicating that they have been trained by the D.O. within the past twelve months. According to the records available on site, Faisal Baz and Lisa Clark were most recently trained on 6-15-09 by the D.O. D.O. employee training must be current for at least one employee on site. Correct immediately. Have the D.O. conduct and document the required training and submit a copy of it to this office. #24 The test results from the annual monitoring system certification testing on 7-16-09 were not received by this office till 9-17-09. Test results must be submitted within 30 days of the test. Begin the practice of ensuring that copies of the test results are submitted within 30 days of the test.
