Lodi, CA 95240 SR0069160 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to install an OPW 2100 series#1 C-21000-DEUR on the 87 <br /> underground storage tank (UST) system at the above noted address. The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the 87 tank prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near the piping, a secondary containment test must be <br /> performed on the piping once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br />
