Please provide the following additional information: <br />1. Primary Containment- Provide cut sheet and independent testing organization (UL) <br />listing for the following: <br />a. OPW shear valves <br />b. Fireflex Flexconnector <br />c. Product piping <br />2. Secondary Contaimnent- Provide cut sheet and Title 23 CCR 2631(d) documentation <br />(independent testing organization or equilvalent) for the following: <br />a. Bravo UDCs <br />b. Bravo penetration fitting F -32 -TS -D <br />3. Provide cut sheet for leak detectors <br />4. Indicate how the double walled UDCs will be monitored, if you are installing double <br />walled UDCs. If not, please update the list of equipment with accurate information <br />5. Technician certifications- UST installer Retrofitter and the following manufacturer <br />certifications: Bravo ( SWAT), Veeder-Root, OPW, etc. <br />6. Complete application <br />a. For page 2, line leak detectors were not mentioned in the scope of work. <br />b. For the site map, Letter P, indicate the existing piping and the UL listing for <br />piping that will be replaced if necessary <br />c. Fill out pages 3-8 for replacing piping and UDCs.
