Financial responsibility documents for 2013 have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually via fax or postal mail. Complete and <br /> submit a copy of the financial responsibility. Submit a written statement how to ensure the financial <br /> responsibility document is submitted to EHD annually. <br /> - Copy of Certification of Financial Responsibility was received on site. It lacked signature & print name <br /> of witness and print name of owner/operator but was corrected on site. <br /> 204. The designated operator Alex Jabbari failed to document reason of alarm and its response to alarm <br /> on the March 2012 designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarms include: L1 87 STP sump Fuel Alarm and L3 91 STP <br /> sump Fuel Alarm both on 03/16/12. Alex documented the alarm but failed to not cause and what actions <br /> were taken in response. He had written an incomplete sentence: "L1: 87 STP sump alarms on 3/16/12 & <br /> 1-3:91 STP sump alarms on 3/16/12 were due to ...". During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented <br /> and responded to appropriately, and attach a copy of the alarm history with documentation taken in <br /> response to any alarms to the monthly report. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports.
