The permit has been approved <br /> with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution <br /> Control District for any applicable permits and requirements. <br /> 2. Contact Muniappa Naidu at(209)468-3439 to arrange the following inspection <br /> appointment(a 48-hour notice is required for inspection appointments): <br /> a. Functionality test to ensure that monitoring has been restored to meet the <br /> requirements of California Code of Regulations Title 23 and the Health& <br /> Safety Code. <br /> a. Cold start has been performed—passed <br /> b. The bravo conversion frame has been verified. <br /> 3. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all <br /> of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. A valid, current California UST Service Technician certificate issued by <br /> the International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired, or serviced. <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br />
