0 0 <br /> The following is an itemized list of underground storage tank violations that have not <br /> been addressed for RAMOS OIL-FRENCH CAMP as of December 28, 2016. <br /> Open violations from October 27, 2016 inspection <br /> Violation #103 -Current financial responsibility documents not submitted. <br /> Current financial responsibility documents are required to be submitted annually. The CFR letter submitted for 2016 <br /> is missing a witness or notary signature. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and <br /> upload the required documents. <br /> This is a repeat violation (which is currently still open)from 2015. <br /> Violation#104-UST operating permit application for facility and tank information not submitted or current. <br /> UST Facility Operating Permit Application and UST Tank Information are not current in CERS. Any change of <br /> information must be updated in CERS within 30 days of the changes. <br /> Missing or incorrect information includes, but may not be limited to: <br /> Piping construction <br /> Vapor recovery construction <br /> Fill components <br /> Corrosion protection <br /> Immediately log into CERS, update the required information, and submit for review by the EHD. <br /> This is a repeat violation (which is currently still open) from 2015. <br /> Violation#105 -Failed to have an approved UST monitoring plan. <br /> The monitoring plan is not current and/or not approved by the EHD. The monitoring plan must be uploaded to the <br /> California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, <br /> and submit for review by the EHD. <br /> Immediately provide a written statement to the EHD indicating what was done to correct this violation. <br /> This is a repeat violation (which is currently still open)from 2015. <br /> Violation#107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations <br /> where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a repeat violation (which is currently still open) from 2015. <br /> Violation#119 -Documents were falsified or misrepresented. <br /> During the inspection it was noted by the inspector several designated monthly inspection reports looked almost <br /> identical to one another. The writing was in red and all of the same fill in the blank areas and "corrections"were <br /> noted. The alarm histories that were attached did not have a date on the top of them. One of the reports from April <br /> should have had an alarm noted and did not. Instead it was noted there had not been any alarms for the previous <br /> month. When the designated operator, Steven Richardson was asked if he had completed the reports today <br /> (October 27, 2016) he said yes. He said the reports had been completed but are now missing. <br /> Immediately prepare a written statement and submit it to the EHD indicating how this practice will be prevented in <br /> the future and whats steps will be taken to ensure compliance. <br /> Page 1 of 5 <br />
