Harlen <br />Rd French Camp site, for every violation I have responded below. Please accept this letter as our <br />return to compliance statement. <br />Violations <br />L(104) Current Financial Responsibility form is attached for your files. Also will be <br />updated on CERS website as well. <br />2.(113) As an error accured on my part I sent the D.O. paperwork to the wrong address. <br />Attached I have photo copied the envelope and D.O. form that I had sent on <br />Sept 9t1i , 2013. 1 did not realize that this happened due to my assistant putting <br />the envelope that I received back in my read box and not into received box. I <br />would have corrected the address and resent back if this did not happen. <br />Attached is the original D.O. form that I sent originally. <br />3.(204) Our D.O. has been counseled to make sure that all forms are filled out correctly <br />and all required info is filled in. <br />P.O.
